You are invited to participate in an online survey.
  This survey is conducted as a part of the project on Means and Methods for Improving Structural Integrity of Roofing Systems.  The project is funded by the Georgeau Construction Research Centre (GCRC) at Western Michigan University and led by Dr.Upul Attanayake, associate professor, with the help of the graduate research assistant Kanchani Basnayake.  The primary goal of the project is to develop a comprehensive research program that allows evaluating various roof systems and materials or mechanisms for improving structural load path integrity and resilience under damaging wind loads.  One of the tasks is to collect information from roofing adhesive manufacturers, product manufacturers, contractors and consultants to understand the industry needs and challenges for product performance evaluation, installation, and handling claims. 

Your participation is voluntary and greatly appreciated.  Each section of the survey should not take more than 25 minutes.
  Privacy of your data

Your survey responses will be stored in a password protected electronic format. No one will be able to identify you or your responses, and no one will know whether or not you participated in the study except the researcher.

At the end of the survey, you will be asked if you are interested in participating in an additional interview (by phone, in person, or email). If you choose to provide contact information, your survey responses may no longer be anonymous to the researcher. However, no names or identifying information would be included in any publications or presentations without your consent and your responses to this survey will remain confidential.
